​Toby is a licensed nursing home administrator with over 20 years in the healthcare industry.  He is passionate about caring for elderly adults.  His devotion for the geriatric population started when he started working in a skilled nursing facility after serving as active duty Air Force Physical Therapy Apprentice.  After several years as a licensed Physical Therapy Assistant, Toby earned a Master’s degree in Healthcare Administration with a focus in Gerontology.  His experience ranges from treating patients, managing the programming and staffing of rehabilitation services at a regional level to leading a home health care agency.   As a leader in the geriatric health care field, he has first-hand appreciation and understanding of the patients and family members journey through the healthcare system.  He continuously helps navigate the continuum of care.

What is the best path?  


One of the most common questions is regarding the appropriate level of care particularly after a being discharged from a hospital or for a person who is slowly becoming less able to maintain living  at home alone.  There are three main categories of care, below we provide a brief description of each.

HOME HEALTH CARE

A wide range of healthcare services are provided at home by licensed professionals, such as nurses, physical and occupational therapist and speech pathologist.

SKILLED NURSING CARE

Patients receive rehabilitation and medical services from nurses, physical and occupational therapists, and speech pathologists.

IN-HOME CARE SERVICES

Provides services at home by persons who are not nurses, doctors, or other licensed medical personnel, as opposed to home health care that is provided by licensed personnel.  Services provided relate to daily living activities, such as meal preparation, light house keeping, errands, companionship and more (depending on the agency).
